Glow in the Dark Dodgeball Registration Form

Each Glow in the Dark Dodgeball team is required to have a team name and have an adult contact person register them via this registration form. They need to have 8 members and be either all boys (Boy's Division) or all girls (Girl's Division). All team members must currently be in the 5th and 6th grade.

The $8 registration fee is due when you sign in your team the night of the event.
